What is Amlodipine 5mg Sandoz?

Amlodipine 5mg Sandoz is used alone or together with other medicines to treat angina (chest pain) and high blood pressure (hypertension). High blood pressure adds to the workload of the heart and arteries. If it continues for a long time, the heart and arteries may not function properly. This can damage the blood vessels of the brain, heart, and kidneys, resulting in a stroke, heart failure, or kidney failure. High blood pressure may also increase the risk of heart attacks. These problems may be less likely to occur if blood pressure is controlled.

Amlodipine 5mg Sandoz is a calcium channel blocker. It affects the movement of calcium into the cells of the heart and blood vessels. As a result, Amlodipine 5mg Sandoz relaxes blood vessels and increases the supply of blood and oxygen to the heart while reducing its workload.

Amlodipine 5mg Sandoz is available only with your doctor's prescription.

Amlodipine 5mg Sandoz indications

An indication is a term used for the list of condition or symptom or illness for which the medicine is prescribed or used by the patient. For example, acetaminophen or paracetamol is used for fever by the patient, or the doctor prescribes it for a headache or body pains. Now fever, headache and body pains are the indications of paracetamol. A patient should be aware of the indications of medications used for common conditions because they can be taken over the counter in the pharmacy meaning without prescription by the Physician.

Hypertension

Amlodipine 5mg Sandoz is indicated for the treatment of hypertension, to lower blood pressure. Lowering blood pressure reduces the risk of fatal and nonfatal cardiovascular events, primarily strokes and myocardial infarctions. These benefits have been seen in controlled trials of antihypertensive drugs from a wide variety of pharmacologic classes including Amlodipine 5mg Sandoz.

Control of high blood pressure should be part of comprehensive cardiovascular risk management, including, as appropriate, lipid control, diabetes management, antithrombotic therapy, smoking cessation, exercise, and limited sodium intake. Many patients will require more than one drug to achieve blood pressure goals. For specific advice on goals and management, see published guidelines, such as those of the National High Blood Pressure Education Program's Joint National Committee on Prevention, Detection, Evaluation, and Treatment of High Blood Pressure (JNC).

Numerous antihypertensive drugs, from a variety of pharmacologic classes and with different mechanisms of action, have been shown in randomized controlled trials to reduce cardiovascular morbidity and mortality, and it can be concluded that it is blood pressure reduction, and not some other pharmacologic property of the drugs, that is largely responsible for those benefits. The largest and most consistent cardiovascular outcome benefit has been a reduction in the risk of stroke, but reductions in myocardial infarction and cardiovascular mortality also have been seen regularly.

Elevated systolic or diastolic pressure causes increased cardiovascular risk, and the absolute risk increase per mmHg is greater at higher blood pressures, so that even modest reductions of severe hypertension can provide substantial benefit. Relative risk reduction from blood pressure reduction is similar across populations with varying absolute risk, so the absolute benefit is greater in patients who are at higher risk independent of their hypertension (for example, patients with diabetes or hyperlipidemia), and such patients would be expected to benefit from more aggressive treatment to a lower blood pressure goal.

Some antihypertensive drugs have smaller blood pressure effects (as monotherapy) in black patients, and many antihypertensive drugs have additional approved indications and effects (e.g., on angina, heart failure, or diabetic kidney disease). These considerations may guide selection of therapy. Amlodipine 5mg Sandoz may be used alone or in combination with other antihypertensive agents.

Coronary Artery Disease (CAD)

Chronic Stable Angina

Amlodipine 5mg Sandoz is indicated for the symptomatic treatment of chronic stable angina. Amlodipine 5mg Sandoz may be used alone or in combination with other antianginal agents.

Vasospastic Angina (Prinzmetal's or Variant Angina)

Amlodipine 5mg Sandoz is indicated for the treatment of confirmed or suspected vasospastic angina. Amlodipine 5mg Sandoz may be used as monotherapy or in combination with other antianginal agents.

Angiographically Documented CAD

In patients with recently documented CAD by angiography and without heart failure or an ejection fraction <40%, Amlodipine 5mg Sandoz is indicated to reduce the risk of hospitalization for angina and to reduce the risk of a coronary revascularization procedure.

This medication is used along with other treatment for certain blood vessel diseases (e.g., arteriosclerosis obliterans, Raynaud's disease, Buerger's disease, cerebrovascular insufficiency). It works by widening blood vessels to help increase blood flow (improve circulation) to certain parts of the body (e.g., hands/feet, brain). This effect may help to decrease symptoms such as cold hands and feet, numbness, tingling, and decreased memory or judgment.

sponsored

Adults

The usual initial antihypertensive oral dose of Amlodipine 5mg Sandoz is 5 mg once daily, and the maximum dose is 10 mg once daily.

Small, fragile, or elderly patients, or patients with hepatic insufficiency may be started on 2.5 mg once daily and this dose may be used when adding Amlodipine 5mg Sandoz to other antihypertensive therapy.

Adjust dosage according to blood pressure goals. In general, wait 7 to 14 days between titration steps. Titrate more rapidly, however, if clinically warranted, provided the patient is assessed frequently.

Angina

The recommended dose for chronic stable or vasospastic angina is 5–10 mg, with the lower dose suggested in the elderly and in patients with hepatic insufficiency. Most patients will require 10 mg for adequate effect.

Coronary Artery Disease

The recommended dose range for patients with coronary artery disease is 5–10 mg once daily. In clinical studies, the majority of patients required 10 mg.

Children

The effective antihypertensive oral dose in pediatric patients ages 6–17 years is 2.5 mg to 5 mg once daily. Doses in excess of 5 mg daily have not been studied in pediatric patients.

Amlodipine 5mg Sandoz has been safely administered with thiazide diuretics, alpha-blockers, beta-blockers, ACE inhibitors, long-acting nitrates, sublingual nitroglycerine, non-steroidal anti-inflammatory drugs, antibiotics, and oral hypoglycemic drugs.

In vitro data from studies with human plasma indicate that Amlodipine 5mg Sandoz has no effect on protein binding of the drugs tested (digoxin, phenytoin, warfarin, or indomethacin).

Simvastatin: Co-administration of multiple doses of 10 mg Amlodipine 5mg Sandoz with 80 mg simvastatin resulted in a 77% increase in exposure to simvastatin compared to simvastatin alone. Limit the dose of simvastatin in patients on Amlodipine 5mg Sandoz to 20 mg daily.

Grapefruit Juice: Co-administration of 240 mL grapefruit juice with a single oral dose of 10 mg Amlodipine 5mg Sandoz in 20 healthy volunteers had no significant effect on the pharmacokinetics of Amlodipine 5mg Sandoz. The study did not allow examination of the effect of genetic polymorphism in CYP3A4, the primary enzyme responsible for metabolism of Amlodipine 5mg Sandoz; therefore, administration of Amlodipine 5mg Sandoz with grapefruit or grapefruit juice is not recommended as bioavailability may be increased in some patients, resulting in increased blood pressure lowering effects.

CYP3A4 Inhibitors: Co-administration of a 180 mg daily dose of diltiazem with 5 mg Amlodipine 5mg Sandoz in elderly hypertensive patients (69 to 87 years of age) resulted in a 57% increase in Amlodipine 5mg Sandoz systemic exposure. Co-administration of erythromycin in healthy volunteers (18 to 43 years of age) did not significantly change Amlodipine 5mg Sandoz systemic exposure (22% increase in area under the concentration versus time curve [AUC]). Although the clinical relevance of these findings is uncertain, pharmacokinetic variations may be more pronounced in the elderly.

Strong inhibitors of CYP3A4 (e.g., ketoconazole, itraconazole, ritonavir) may increase the plasma concentrations of Amlodipine 5mg Sandoz to a greater extent than diltiazem. Amlodipine 5mg Sandoz should be used with caution when administered with CYP3A4 inhibitors.

Clarithromycin: Clarithromycin is an inhibitor of CYP3A4. There is an increased risk of hypotension in patients receiving clarithromycin with Amlodipine 5mg Sandoz. Close observation of patients is recommended when Amlodipine 5mg Sandoz is co-administered with clarithromycin.

CYP3A4 Inducers: There is no data available regarding the effect of CYP3A4 inducers on Amlodipine 5mg Sandoz. Concomitant use of CYP3A4 inducers (e.g., rifampicin, Hypericum perforatum) may decrease the plasma concentrations of Amlodipine 5mg Sandoz. Amlodipine 5mg Sandoz should be used with caution when administered with CYP3A4 inducers.

In the following studies, there were no significant changes in the pharmacokinetics of either Amlodipine 5mg Sandoz or another drug within the study, when co-administered.

Special Studies: Effect of Other Agents on Amlodipine 5mg Sandoz: Cimetidine: Co-administration of Amlodipine 5mg Sandoz with cimetidine did not alter the pharmacokinetics of Amlodipine 5mg Sandoz.

Aluminum/Magnesium (Antacid): Co-administration of aluminum/magnesium (antacid) with a single dose of Amlodipine 5mg Sandoz had no significant effect on the pharmacokinetics of Amlodipine 5mg Sandoz.

Sildenafil: A single 100 mg dose of sildenafil in subjects with essential hypertension had no effect on the pharmacokinetic parameters of Amlodipine 5mg Sandoz. When Amlodipine 5mg Sandoz and sildenafil were used in combination, each agent independently exerted its own blood pressure lowering effect.

Special Studies: Effect of Amlodipine 5mg Sandoz on Other Agents: Atorvastatin: Co-administration of multiple 10 mg doses of Amlodipine 5mg Sandoz with 80 mg atorvastatin resulted in no significant change in the steady-state pharmacokinetic parameters of atorvastatin.

Digoxin: Co-administration of Amlodipine 5mg Sandoz with digoxin did not change serum digoxin levels or digoxin renal clearance in healthy volunteers.

Ethanol (Alcohol): Single and multiple 10 mg doses of Amlodipine 5mg Sandoz had no significant effect on the pharmacokinetics of ethanol.

Warfarin: Co-administration of Amlodipine 5mg Sandoz with warfarin did not change the warfarin prothrombin response time.

Cyclosporin: Pharmacokinetic studies with cyclosporin have demonstrated that Amlodipine 5mg Sandoz does not significantly alter the pharmacokinetics of cyclosporin.

Tacrolimus: There is a risk of increased tacrolimus blood levels when co-administered with Amlodipine 5mg Sandoz. In order to avoid toxicity of tacrolimus, administration of Amlodipine 5mg Sandoz in a patient treated with tacrolimus requires monitoring of tacrolimus blood levels and dose adjustment of tacrolimus when appropriate.

Drug/Laboratory Test Interactions: None known.

Clinical Trials Experience

Because clinical trials are conducted under widely varying conditions, adverse reaction rates observed in the clinical trials of a drug cannot be directly compared to rates in the clinical trials of another drug and may not reflect the rates observed in practice.

Amlodipine 5mg Sandoz has been evaluated for safety in more than 11,000 patients in U.S. and foreign clinical trials. In general, treatment with Amlodipine 5mg Sandoz was well-tolerated at doses up to 10 mg daily. Most adverse reactions reported during therapy with Amlodipine 5mg Sandoz were of mild or moderate severity. In controlled clinical trials directly comparing Amlodipine 5mg Sandoz (N=1730) at doses up to 10 mg to placebo (N=1250), discontinuation of Amlodipine 5mg Sandoz because of adverse reactions was required in only about 1.5% of patients and was not significantly different from placebo (about 1%). The most commonly reported side effects more frequent than placebo are reflected in the table below. The incidence (%) of side effects that occurred in a dose related manner are as follows:

2.5 mg Amlodipine 5mg Sandoz 5 mg 10 mg Placebo
