The dose which the patient gets through the mouthpiece of the device is 180 mcg.

Beclometasone Topical (Dermolin GM) contains 200 doses of finely powdered beclomethasone dipropionate mixed in a small quantity of lactose. The amount of drug received by the patient (180 mcg/dose) corresponds to that from 2 puffs of the freon-based beclomethasone inhalation aerosol with a metered dose of 100 mcg/dose. The dose to be inhaled is released from the container by pressing the device between the thumb and the forefinger. This is followed by inhalation through the device. As a result, beclomethasone dipropionate particles are transferred to their target, the lungs. Use of the Beclometasone Topical (Dermolin GM) does not require coordination of actuation of the dose and inhalation.

Management of Chronic Asthma: Adults and Children >12 years: 100-400 mcg twice daily. Maximum: 1000 mcg twice daily.

Children (5-12 years): 100-200 mcg twice daily. Maximum: 200-400 mcg twice daily.

Prophylaxis of Asthma: Adults and Children >12 years: 50-200 mcg twice daily. Increase if necessary to maximum 400 mcg twice daily.

Children (5-12 years): The usual starting dose is 100 mcg twice daily. Depending on the severity of asthma, the daily dose may be increased up to 400 mcg administered in 2-4 divided doses.

When patient's symptoms remain under satisfactory control, the dose should be titrated to the lowest dose at which effective control of asthma is maintained.

Therapy in New Patients: Mild Asthma: 50-100 mcg twice daily.

Moderate Asthma: 100-200 mcg twice daily.

Severe Asthma: 200-400 mcg twice daily.

When switching a patient with well-controlled asthma from another corticosteroid inhaler, initially Beclometasone Topical (Dermolin GM) 100 mcg puff should be prescribed for beclometasone dipropionate (CFC) or budesonide 200-250 mcg; fluticasone propionate 100 mcg.

When switching a patient with poorly-controlled asthma from another corticosteroid inhaler, initially Beclometasone Topical (Dermolin GM) 100 mcg puff should be prescribed for beclometasone dipropionate (CFC) or budesonide or fluticasone propionate 100 mcg. The dose of Beclometasone Topical (Dermolin GM) should be adjusted to response.

Use in the elderly or patients with hepatic or renal impairment: There are no special dosage recommendations for use in the elderly or in patients with hepatic or renal impairment.

Due to the very low plasma concentration achieved after inhaled dosing, clinically significant drug interactions are in general unlikely. Care should be taken when co-administering with known strong CYP3A4 inhibitors (eg, ketoconazole, itraconazole, nelfinavir, ritonavir) as there is a potential for increased systemic exposure to beclometasone.

Clotrimazole Topical (Dermolin GM) Cream: Clotrimazole Topical (Dermolin GM) cream is an antifungal cream used to treat skin infections caused by dermatophytes, fungi, yeasts, moulds, etc [eg, athlete's foot, jock itch, ringworm of the body, cutaneous candidiasis (candida infection of the skin which may occur in diaper areas in babies, in armpits, groins or areas under the breasts)], pityriasis versicolor (commonly called white spots).

Clotrimazole Topical (Dermolin GM) cream can also be used for erythrasma (a skin infection caused by the bacteria Corynebacterium minutissimum and usually affects areas with skin folds eg, the groin, armpits, between toes).

Clotrimazole Topical (Dermolin GM) cream can also be used for female fungal infections of the vaginal area. It can also be used in males where inflammation is present in the penis, caused by yeast fungi (Candida vulvitis and Candida balanitis).

Topical Powder:

Skin infections caused by fungi (eg, dermatophytes, yeasts, moulds) is treated with an antifungal product (eg, Clotrimazole Topical (Dermolin GM) cream).

Clotrimazole Topical (Dermolin GM) powder may be used as an adjuvant to the previously mentioned treatment or as a follow-up treatment (prevention of reinfection) of the previously mentioned fungal infections (dermatomycoses) and erythrasma.

Clotrimazole Topical (Dermolin GM)® Troches are administered only as a lozenge that must be slowly dissolved in the mouth. The recommended dose is one troche five times a day for fourteen consecutive days. Only limited data are available on the safety and effectiveness of the clotrimazole troche after prolonged administration; therefore, therapy should be limited to short term use, if possible.

For prophylaxis to reduce the incidence of oropharyngeal candidiasis in patients immunocompromised by conditions that include chemotherapy, radiotherapy, or steroid therapy utilized in the treatment of leukemia, solid tumors, or renal transplantation, the recommended dose is one troche three times daily for the duration of chemotherapy or until steroids are reduced to maintenance levels.

Renal Dose Adjustments

Patients with impaired renal function have a high risk of developing nephrotoxicity and ototoxicity with oral Neomycin Topical (Dermolin GM). Use of a less nephrotoxic medication may be a consideration for this patient.

Liver Dose Adjustments

In chronic hepatic insufficiency, it may be necessary to give Neomycin Topical (Dermolin GM) sulfate 4 g daily for an indefinite period, if less toxic drugs cannot be used.

Dose Adjustments

Caution should be used when administering Neomycin Topical (Dermolin GM) to patients with renal impairment. Dose reduction should be considered in these patients.

Precautions

To minimize the risk of toxicity use the lowest possible dose and the shortest possible treatment period to control the condition. Treatment periods of longer than 2 weeks are not recommended.

Do not use in patients with intestinal obstruction.

Do not use in patients with inflammatory or ulcerative gastrointestinal disease due to the potential for increased gastrointestinal absorption of the drug.

Patients should be monitored for nephrotoxicity and ototoxicity.

Neomycin Topical (Dermolin GM) should be used with caution in patients with hearing impairment, renal impairment or neuromuscular disorders.

Dialysis

Patients with impaired renal function have a high risk of developing nephrotoxicity and ototoxicity with oral Neomycin Topical (Dermolin GM). Use of a less nephrotoxic medication may be a consideration for this patient.

Oral Neomycin Topical (Dermolin GM) inhibits the gastrointestinal absorption of penicillin V, oral vitamin B-12, methotrexate and 5-fluorouracil. The gastrointestinal absorption of digoxin also appears to be inhibited. Therefore, digoxin serum levels should be monitored.

Oral Neomycin Topical (Dermolin GM) sulfate may enhance the effect of coumarin in anticoagulants by decreasing vitamin K availability.
