An alkaloid from Solanaceae, especially Datura metel L. and Scopola carniolica. Emes and its quaternary derivatives act as antimuscarinics like atropine, but may have more central nervous system effects. Among the many uses are as an anesthetic premedication, in urinary incontinence, in motion sickness, as an antispasmodic, and as a mydriatic and cycloplegic.

Usual Adult Dose for Motion Sickness

-IM: 50 mg every 4 hours injected intramuscularly as needed; Maximum Dose: 100 mg every 4 hours

-IV: 50 mg every 4 hours injected intravenously over a period of 2 minutes; Maximum Dose: 100 mg every 4 hours

-Oral: 50 to 100 mg orally every 4 to 6 hours; Maximum Dose: 400 mg in 24 hours

-For IV administration: Each mL (50 mg) must be diluted in 10 mL of 0.9% sodium chloride injection.

-The first dose should be taken one-half to one hour before starting activity.

-The injectable form is indicated when the oral form is impractical.

Use: Prevention and treatment of nausea, vomiting, dizziness, or vertigo associated with motion sickness.

Usual Pediatric Dose for Motion Sickness

IM: 1.25 mg/kg OR 37.5 mg/m2 injected intramuscularly 4 times a day; Maximum Dose: 300 mg/day

Oral:

-Age 2 to 5: 12.5 to 25 mg orally every 6 to 8 hours; Maximum Dose: 75 mg in 24 hours

-Age 6 to 11: 25 to 50 mg orally every 6 to 8 hours; Maximum Dose: 150 mg in 24 hours

-Age 12 and older: 50 to 100 mg orally every 4 to 6 hours; Maximum Dose: 400 mg in 24 hours

Administration Advice:

-The solution form should not be injected intra-arterially.

Storage Requirements:

-This drug should be protected from moisture and stored at 20 to 25 degrees Celsius (68 to 77 Fahrenheit).

General:

-Overdosage: Convulsions, coma, respiratory depression, and drowsiness may occur with overdosage. There is no specific antidote; the manufacturer product information should be consulted for drugs that may be used to treat overdosage effects.

-In mice, the oral LD50 is 203 mg/kg and the intraperitoneal LD50 is 149 mg/kg; the LD50 in rats is 1,320 mg/kg.

-There is no human data indicating this drug is a carcinogen or mutagen.

Patient Advice:

-Avoid alcoholic drinks, sedatives, and tranquilizers while taking this drug.

-This drug may cause side effects such as drowsiness and dizziness that can affect your ability to perform certain activities; avoid driving and potentially hazardous activities such as operating machinery until you know how this drug affects you.

The absorption of oral medications may be decreased during the concurrent use of scopolamine because of decreased gastric motility and delayed gastric emptying.

Emes should be used with care in patients taking other drugs that are capable of causing CNS effects such as sedatives, tranquilizers, or alcohol. Special attention should be paid to potential interactions with drugs having anticholinergic properties; e.g., other belladonna alkaloids, antihistamines (including meclizine), tricyclic antidepressants, and muscle relaxants.

Side effects include:

Drowsiness, headache, blurred vision, tinnitus, dryness of the mouth and respiratory passages, incoordination, palpitation, dizziness, hypotension.

Paradoxical CNS stimulation in pediatric patients and occasionally in adults.

With IM injection: Pain at the injection site.

Because Emes (the active ingredient contained in Emes) contains diphenhydramine, the possibility of other diphenhydramine-related adverse effects should also be considered.
