Actions of Calcium Carbonate (Shelcal-HD) in details

The action of the drug on the human body is called Pharmacodynamics in Medical terminology. To produce its effect and to change the pathological process that is happening the body and to reduce the symptom or cure the disease, the medicine has to function in a specific way. The changes it does to the body at cellular level gives the desired result of treating a disease. Drugs act by stimulating or inhibiting a receptor or an enzyme or a protein most of the times. Medications are produced in such a way that the ingredients target the specific site and bring about chemical changes in the body that can stop or reverse the chemical reaction which is causing the disease.

Calcium Carbonate (Shelcal-HD) is a basic inorganic salt that acts by neutralizing hydrochloric acid in gastric secretions. It also inhibits the action of pepsin by increasing the pH and via adsorption. Cytoprotective effects may occur through increases in bicarbonate ion (HCO3-) and prostaglandins. Neutralization of hydrochloric acid results in the formation of calcium chloride, carbon dioxide and water. Approximately 90% of calcium chloride is converted to insoluble calcium salts (e.g. Calcium Carbonate (Shelcal-HD) and calcium phosphate).

How should I take Calcium Carbonate (Shelcal-HD)?

Take Calcium Carbonate (Shelcal-HD) exactly as directed by your doctor or follow the directions on the package. If you do not understand these directions, ask your pharmacist, nurse, or doctor to explain them to you.

Swallow the Calcium Carbonate (Shelcal-HD) tablets and capsules with a full glass of water.

Chew the chewable forms of Calcium Carbonate (Shelcal-HD) completely before swallowing.

Use the Calcium Carbonate (Shelcal-HD) powder as directed. Allow the powder to dissolve completely, then consume the mixture.

Shake the Calcium Carbonate (Shelcal-HD) suspension well before measuring a dose. To ensure that you get the correct dose, use a dose-measuring spoon or cup, not a regular table spoon to measure the liquid. If you do not have a dose-measuring device, ask your pharmacist where you can get one.

Take calcium with meals to increase its absorption by the body, unless otherwise directed by your doctor.

Store Calcium Carbonate (Shelcal-HD) at room temperature away from moisture and heat.

Calcium Carbonate (Shelcal-HD) administration

Administration of drug is important to know because the drug absorption and action varies depending on the route and time of administration of the drug. A medicine is prescribed before meals or after meals or along with meals. The specific timing of the drug intake about food is to increase its absorption and thus its efficacy. Few work well when taken in empty stomach and few medications need to be taken 1 or 2 hrs after the meal. A drug can be in the form of a tablet, a capsule which is the oral route of administration and the same can be in IV form which is used in specific cases. Other forms of drug administration can be a suppository in anal route or an inhalation route.

Oral: Administer with food. Doses >600 mg (

elemental calcium) per day should be divided for optimal absorption (AACE [Camacho 2016]).

Capsules may be swallowed whole or opened and the contents mixed with food or drink.

Calcium Carbonate (Shelcal-HD) pharmacology

Pharmacokinetics of a drug can be defined as what body does to the drug after it is taken. The therapeutic result of the medicine depends upon the Pharmacokinetics of the drug. It deals with the time taken for the drug to be absorbed, metabolized, the process and chemical reactions involved in metabolism and about the excretion of the drug. All these factors are essential to deciding on the efficacy of the drug. Based on these pharmacokinetic principles, the ingredients, the Pharmaceutical company decides dose and route of administration. The concentration of the drug at the site of action which is proportional to therapeutic result inside the body depends on various pharmacokinetic reactions that occur in the body.

Gastric-peptic disease occurs as a result of an imbalance between protective factors, such as mucus, bicarbonate, and prostaglandin secretion, and aggressive factors, such as hydrochloric acid, pepsin, and Helicobacter pylori (H. pylori). Antacids work by restoring acid-base balance, attenuating the pepsin activity and increasing bicarbonate and prostaglandin secretion. The acid-neutralizing capacity of Calcium Carbonate (Shelcal-HD) is 58 mEq/15 ml. When used as a nutritional supplement, Calcium Carbonate (Shelcal-HD) acts by directly increasing calcium stores within the body.

Vitamin D3 (Shelcal-HD) increases the absorption of calcium and phosphorous that are vital in forming and maintaining strong bones. It is also required in regulating cell growth and maintaining a healthy immune system.

Follow all directions on your prescription label. Your doctor may occasionally change your dose to make sure you get the best results. Do not take this medicine in larger or smaller amounts or for longer than recommended.

Measure liquid medicine with the dosing syringe provided, or with a special dose-measuring spoon or medicine cup. If you do not have a dose-measuring device, ask your pharmacist for one.

The chewable tablet must be chewed before you swallow it.

The cholecalciferol wafer is usually taken only once per week or once per month. Follow your doctor's dosing instructions very carefully. The wafer must be chewed before you swallow it.

Cholecalciferol is only part of a complete program of treatment that may also include a special diet. It is very important to follow the diet plan created for you by your doctor or nutrition counselor. You should become very familiar with the list of foods you must eat or avoid to help control your condition.

Store at room temperature away from moisture, light, and heat.

The first step involved in the activation of Vitamin D3 (Shelcal-HD) is a 25-hydroxylation which is catalysed by the 25-hydroxylase in the liver and then by other enzymes. The mitochondrial sterol 27-hydroxylase catalyses the first reaction in the oxidation of the side chain of sterol intermediates. The active form of Vitamin D3 (Shelcal-HD) (calcitriol) binds to intracellular receptors that then function as transcription factors to modulate gene expression. Like the receptors for other steroid hormones and thyroid hormones, the vitamin D receptor has hormone-binding and DNA-binding domains. The vitamin D receptor forms a complex with another intracellular receptor, the retinoid-X receptor, and that heterodimer is what binds to DNA. In most cases studied, the effect is to activate transcription, but situations are also known in which vitamin D suppresses transcription. Calcitriol increases the serum calcium concentrations by: increasing GI absorption of phosphorus and calcium, increasing osteoclastic resorption, and increasing distal renal tubular reabsorption of calcium. Calcitriol appears to promote intestinal absorption of calcium through binding to the vitamin D receptor in the mucosal cytoplasm of the intestine. Subsequently, calcium is absorbed through formation of a calcium-binding protein.
