Caffeine (Sydolil) is a stimulant that causes narrowing of blood vessels (vasoconstriction).
Ergotamine (Sydolil) is in a group of drugs called ergot alkaloids (ER-got AL-ka-loids). It works by narrowing the blood vessels around the brain. Ergotamine (Sydolil) also affects blood flow patterns that are associated with certain types of headaches.
Caffeine (Sydolil) and Ergotamine (Sydolil) is a combination medicine used to treat or prevent a migraine type headache.
This medication will only treat a headache that has already begun. It will not prevent migraine headaches or reduce the number of attacks.
Caffeine (Sydolil) and Ergotamine (Sydolil) should not be used to treat common tension headaches or any headache that seems to be different from your usual migraine headaches.
Caffeine (Sydolil) and Ergotamine (Sydolil) may also be used for purposes not listed in this medication guide.

How should I use Sydolil?

Sydolil suppositories may be taken with or without food.
Eating grapefruit or drinking grapefruit juice may affect the amount of Sydolil suppositories in your blood. Talk with your doctor before including grapefruit or grapefruit juice in your diet.
Sydolil suppositories is only to be used as needed for a migraine attack.
Use Sydolil suppositories at the first sign of a headache. Remove the wrapper and moisten the suppository with cool water. Lie down on your side. Insert the pointed end of the suppository into the rectum, then use your finger to push it in completely. Wash your hands thoroughly after using Sydolil suppositories.
If the suppository has softened because of a period of unavoidable exposure to heat, place in the refrigerator for about 15 minutes or chill in ice cold water before removing the wrapper.
If the initial dose does not relieve your headache, an additional dose may be needed according to your doctor's instructions. Do not use a dose of Sydolil suppositories within 1 hour of your last dose.
Do not use more than 2 suppositories for a single migraine headache or more than 5 suppositories during any 7-day period without first checking with your doctor.
If you miss a dose of Sydolil suppositories and you still have a headache, use it as soon as you remember. Do not use a dose of Sydolil suppositories within 1 hour of your last dose. Do not use 2 doses at once.

How to use Sydolil
Take this medication by mouth with or without food as directed by your doctor, usually 2 tablets at the first sign of headache, followed by 1 tablet every half hour until the headache is relieved. Do not take more than 6 tablets per headache attack or 10 tablets in a 7-day period.
If your doctor has prescribed this medication to help prevent certain types of headaches (such as cluster headaches), take this medication exactly as directed by your doctor, usually for short-term treatment (2 to 3 weeks).
The dosage is based on your medical condition and response to treatment. This medication works best if it is taken at the first sign of the headache. If you wait until the headache has worsened, the medication may not work as well.
If you have a higher risk for heart problems, your doctor may perform a heart exam before you start taking this medication. He/she may also direct you to take your first dose of this medication in the office/clinic to monitor for serious side effects (such as chest pain). Talk to your doctor for details.
This medication is usually taken only as needed. It is not meant for long-term daily use.
Along with its benefits, this medication may rarely cause abnormal drug-seeking behavior (addiction). This risk may be increased if you have abused alcohol or drugs in the past. Take this medication exactly as prescribed to lessen the risk of addiction.
Overuse of drugs to treat sudden migraine attacks can sometimes lead to worsening of headache (medication overuse headache) or rebound headache. Therefore, do not use this medication more often or for longer than prescribed. Tell your doctor if you need to use this medication more often, if the medication is not working as well, or if you have more frequent or worse headaches. Your doctor may need to change your medication and/or add another medication to help prevent headaches.

Two suppositories is the maximum dose for an individual attack.

CYP 3A4 Inhibitors (e.g., Macrolide Antibiotics and Protease Inhibitors): See CONTRAINDICATIONS and WARNINGS.
Sydolil® (Ergotamine (Sydolil) tartrate and Caffeine (Sydolil) tablets, USP) should not be administered with other vasoconstrictors. Use with sympathomimetics (pressor agents) may cause extreme elevation of blood pressure. The beta-blocker Inderal (propranolol) has been reported to potentiate the vasoconstrictive action of Sydolil® (Ergotamine (Sydolil) tartrate and Caffeine (Sydolil) tablets, USP) by blocking the vasodilating property of epinephrine. Nicotine may provoke vasoconstriction in some patients, predisposing to a greater ischemic response to ergot therapy.
The blood levels of Ergotamine (Sydolil)-containing drugs are reported to be elevated by the concomitant administration of macrolide antibiotics and vasospastic reactions have been reported with therapeutic doses of the Ergotamine (Sydolil)-containing drugs when coadministered with these antibiotics.
Cardiovascular: Vasoconstrictive complications of a serious nature may occur at times. These include ischemia, cyanosis, absence of pulse, cold extremities, gangrene, precordial distress and pain, EKG changes and muscle pains. Although these effects occur most commonly with long-term therapy at relatively high doses, they have also been reported with short-term or normal doses. Other cardiovascular adverse effects include transient tachycardia or bradycardia and hypertension.
Gastrointestinal: Nausea and vomiting.
Neurological: paresthesias, numbness, weakness, and vertigo.
Allergic: Localized edema and itching.
Drug Abuse And Dependence: There have been reports of drug abuse and psychological dependence in patients on Sydolil® (Ergotamine (Sydolil) tartrate and Caffeine (Sydolil)) (Ergotamine (Sydolil) tartrate and Caffeine (Sydolil) tablets, USP) therapy. Due to the chronicity of vascular headaches, it is imperative that patients be advised not to exceed recommended dosages with long-term use to avoid ergotism.
Coadministration of Ergotamine (Sydolil) with potent CYP 3A4 inhibitors (ritonavir, nelfinavir, indinavir, erthromycin, clarithromycin, and troleandomycin) has been associated with acute ergot toxicity (ergotism) characterized by vasospasm and ischemia of the extremities, with some cases resulting in amputation. There have been rare reports of cerebral ischemia in patients on protease inhibitor therapy when Sydolil® (Ergotamine (Sydolil) tartrate and Caffeine (Sydolil)) (Ergotamine (Sydolil) tartrate and Caffeine (Sydolil) tablets, USP) was coadministered, at least one resulting in death. Because of the increased risk for ergotism and other serious vasospastic adverse events, Ergotamine (Sydolil) use is contraindicated with these drugs and other potent inhibitors of CYP 3A4 (e.g., ketoconazole, itraconazole).
Sydolil® (Ergotamine (Sydolil) tartrate and Caffeine (Sydolil) tablets, USP) may cause fetal harm when administered to pregnant women. Sydolil® (Ergotamine (Sydolil) tartrate and Caffeine (Sydolil)) (Ergotamine (Sydolil) tartrate and Caffeine (Sydolil) tablets, USP) is contraindicated in women who are or may become pregnant. If this drug is used during pregnancy or if the patient becomes pregnant while taking this product, the patient should be apprised of the potential hazard to the fetus.
