What is Depram?
Depram is a tricyclic antidepressant. Depram affects chemicals in the brain that may become unbalanced.
Depram is used to treat symptoms of depression. Depram is sometimes used to treat bed-wetting in children ages 6 and older.
Depram may also be used for purposes not listed in this medication guide.
Depram indications
Depression
For the relief of symptoms of depression. Endogenous depression is more likely to be alleviated than other depressive states. One to three weeks of treatment may be needed before optimal therapeutic effects are evident.
Childhood Enuresis
May be useful as temporary adjunctive therapy in reducing enuresis in children aged 6 years and older, after possible organic causes have been excluded by appropriate tests. In patients having daytime symptoms of frequency and urgency, examination should include voiding cystourethrography and cystoscopy, as necessary. The effectiveness of treatment may decrease with continued drug administration.
How should I use Depram?
Use Depram capsules as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- Depram capsules comes with an extra patient information sheet called a Medication Guide. Read it carefully. Read it again each time you get Depram capsules refilled.
- Take Depram capsules by mouth with or without food.
- Taking Depram capsules at bedtime may help reduce side effects (eg, daytime drowsiness). Discuss this possibility with your doctor.
- It may take 1 to 3 weeks before you notice the effect of Depram capsules. Continue to use Depram capsules even if you feel well. Do not miss any doses.
- If you miss a dose of Depram capsules, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once. If you take 1 dose daily at bedtime, do not take the missed dose the next morning.

Off Label Uses
Bulimia nervosa
Data from double-blind placebo-controlled trials support the use of Depram in the treatment of bulimia nervosa; however, all studies were limited by reliance on self-reporting of eating behavior.
The American Psychiatric Association guidelines for the treatment of patients with eating disorders indicate that tricyclic antidepressants may be a second-line option in patients who are at low risk for suicide. According to World Federation of Societies of Biological Psychiatry guidelines for the pharmacological treatment of eating disorders, Depram is effective and recommended in the management of bulimia nervosa.
Neuropathic pain
Data from two randomized, double-blind, placebo-controlled, crossover trials.
Depram description
Depram, the prototypical tricyclic antidepressant (TCA), is a dibenzazepine-derivative TCA. TCAs are structurally similar to phenothiazines. They contain a tricyclic ring system with an alkyl amine substituent on the central ring. In non-depressed individuals, Depram does not affect mood or arousal, but may cause sedation. In depressed individuals, Depram exerts a positive effect on mood. TCAs are potent inhibitors of serotonin and norepinephrine reuptake. Tertiary amine TCAs, such as Depram and amitriptyline, are more potent inhibitors of serotonin reuptake than secondary amine TCAs, such as nortriptyline and desipramine. TCAs also down-regulate cerebral cortical β-adrenergic receptors and sensitize post-synaptic serotonergic receptors with chronic use. The antidepressant effects of TCAs are thought to be due to an overall increase in serotonergic neurotransmission. TCAs also block histamine H1 receptors, α1-adrenergic receptors and muscarinic receptors, which accounts for their sedative, hypotensive and anticholinergic effects (e.g. blurred vision, dry mouth, constipation, urinary retention), respectively. Depram has less sedative and anticholinergic effects than the tertiary amine TCAs, amitriptyline and clomipramine. See toxicity section below for a complete listing of side effects. Depram may be used to treat depression and nocturnal enuresis in children. Unlabeled indications include chronic and neuropathic pain (including diabetic neuropathy), panic disorder, attention-deficit/hyperactivity disorder (ADHD), and post-traumatic stress disorder (PTSD).

Initially, up to 100 mg/day intramuscularly in divided doses.
Parenteral administration should be used only for starting therapy in patients unable or unwilling to use oral medication. The oral form should supplant the injectable as soon as possible.
Lower dosages are recommended for elderly patients and adolescents. Lower dosages are also recommended for outpatients as compared to hospitalized patients who will be under close supervision. Dosage should be initiated at a low level and increased gradually, noting carefully the clinical response and any evidence of intolerance. Following remission, oral maintenance medication may be required for a longer period of time, at the lowest dose that will maintain remission.

If Apo-Depram is taken with certain other drugs, the effects of either could be increased, decreased, or altered. Patient should check with doctor before combining Apo-Depram with the following

If any of the following side effects occur while taking Depram, check with your doctor immediately:

- burning, crawling, itching, numbness, prickling, "pins and needles", or tingling feelings
- chest pain or discomfort
- clay-colored stools
- cold sweats
- confusion about identity, place, and time

- dark urine
- decrease in the frequency of urination
- difficulty in passing urine (dribbling)
- dizziness, faintness, or lightheadedness when getting up suddenly from a lying or sitting position

- fast, pounding, or irregular heartbeat or pulse
- feeling, seeing, or hearing things that are not there
- feeling that others are watching you or controlling your behavior
- feeling that others can hear your thoughts
- fever with or without chills
- flushed, dry skin
- general feeling of tiredness or weakness
- hearing loss
- hostility
- hyperventilation
- inability to move the arms, legs, or facial muscles
- irritability
- itching or rash
- lack of coordination
- lethargy

- lower back or side pain

- muscle spasm or jerking of all extremities

- weakness in the arms, hands, legs, or feet
- yellow eyes or skin
If any of the following symptoms of overdose occur while taking Depram, get emergency help immediately:
Symptoms of overdose:
- Bluish color of fingernails, lips, skin, palms, or nail beds
- cold, clammy skin
- decreased awareness or responsiveness
- difficult or troubled breathing
- disorientation
- fast, weak pulse
- hallucinations
- irregular, fast, slow, or shallow breathing
- severe sleepiness
Minor Side Effects
Some Depram side effects may not need any medical attention. As your body gets used to the medicine these side effects may disappear. Your health care professional may be able to help you prevent or reduce these side effects, but do check with them if any of the following side effects continue, or if you are concerned about them:

- Bigger, dilated, or enlarged pupils (black part of the eye)
- black tongue
- decreased interest or ability in sexual intercourse
- difficulty having a bowel movement (stool)
- enlargement of the breast
- hives or welts
- increase in sexual ability, desire, drive, or performance
- increased sensitivity of the eyes to light
- increased urge to urinate during the night
- peculiar taste
- redness or other discoloration of the skin
- severe sunburn
- swelling of the testicles
- swelling of the breasts or breast soreness in both females and males
- swollen, painful, or tender lymph glands on the side of the face or neck
- unexpected or excess milk flow from the breasts
- waking to urinate at night

Monoamine Oxidase Inhibitors (MAOIs)
The use of MAOIs intended to treat psychiatric disorders with Depram Pamoate or within 14 days of stopping treatment with Depram Pamoate is contraindicated because of an increased risk of serotonin syndrome. The use of Depram Pamoate within 14 days of stopping an MAOI intended to treat psychiatric disorders is also contraindicated.
Starting Depram Pamoate in a patient who is being treated with MAOIs such as linezolid or intravenous methylene blue is also contraindicated because of an increased risk of serotonin syndrome.
Myocardial Infarction
The drug is contraindicated during the acute recovery period after a myocardial infarction.
Hypersensitivity to Tricyclic Antidepressants
Patients with a known hypersensitivity to this compound should not be given the drug. The possibility of cross-sensitivity to other dibenzazepine compounds should be kept in mind.
