What is Gonapure?
Gonapure are a mixture of follicle-stimulating hormone (FSH) and luteinizing hormone (LH). FSH and LH are important in the development of follicles (eggs) that are produced by the ovaries in women.
Gonapure are used to help your body produce multiple eggs during ovulation, in preparation for in-vitro fertilization.
Gonapure may also be used for purposes not listed in this medication guide.
Gonapure indications
Menotrophin, in conjunction with hCG, is indicated for multiple follicular development (controlled ovarian stimulation) and ovulation induction in patients who have previously received pituitary suppression.
Selection of Patients
1.Before treatment with Menotrophin is instituted, a thorough gynecologic and endocrinologic evaluation must be performed. Except for those patients enrolled in an in vitrofertilization program, this should include a hysterosalpingogram (to rule out uterine and tubal pathology) and documentation of anovulation by means of basal body temperature, serial vaginal smears, examination of cervical mucus, determination of serum (or urine) progesterone, urinary pregnanediol and endometrial biopsy. Patients with tubal pathology should receive Gonapure only if enrolled in an in vitro fertilization program.
2.Primary ovarian failure should be excluded by the determination of gonadotropin levels.
3.Careful examination should be made to rule out the presence of an early pregnancy.
4.Patients in late reproductive life have a greater predilection to endometrial carcinoma as well as a higher incidence of anovulatory disorders. Cervical dilation and curettage should always be done for diagnosis before starting Menotrophin therapy in such patients who demonstrate abnormal uterine bleeding or other signs of endometrial abnormalities.
5.Evaluation of the husband’s fertility potential should be included in the workup.
How should I use Gonapure?
Use Gonapure as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- Gonapure is administered as an injection either under the skin (subcutaneously) or into a muscle (intramuscularly). If you are using Gonapure at home, carefully follow the injection procedures taught to you by your health care provider.
- Do not use Gonapure if it contains particles, is cloudy or discolored, or if the vial is cracked or damaged.
- Keep this product, as well as syringes and needles, out of the reach of children and away from pets. Do not reuse needles, syringes, or other materials. Ask your health care provider how to dispose of these materials after use. Follow all local rules for disposal.
- If you miss a dose of Gonapure, contact your doctor to find out what to do.

Uses of Gonapure in details
Use: Labeled Indications
For multiple follicle development and pregnancy in ovulatory women as part of an assisted reproductive technology cycle.
Limitations of use: Prior to therapy, preform a complete gynecologic exam and endocrinologic evaluation to diagnose the cause of infertility; exclude the possibility of pregnancy; evaluate the fertility status of the male partner; exclude a diagnosis of primary ovarian failure.
Off Label Uses
Stimulation of spermatogenesis
Based on the American Association of Clinical Endocrinologists medical guidelines for clinical practice for the evaluation and treatment of hypogonadism in adult male patients, Gonapure given for stimulation of spermatogenesis in males with primary or secondary hypogonadotropic hypogonadism is effective and recommended in the management of this condition.

Injection, powder for reconstitution:
Menopur: 75 units [supplied with diluent]
Dosing: Adult
Assisted reproductive technologies (females): SubQ: Initial: 225 units once daily beginning on cycle day 2 or 3; Gonapure may be administered together with urofollitropin and the total initial dose of both products combined should not exceed 225 units (Gonapure 150 units and urofollitropin 75 units; or Gonapure 75 units and urofollitropin 150 units). Adjust dose after 5 days based on ultrasound monitoring of ovarian response and/or measurement of serum estradiol levels. Do not make additional adjustments more frequently than once every 2 days or by >150 units. Maximum daily dose: 450 units (of Gonapure, or Gonapure plus urofollitropin); treatment >20 days is not recommended. Once follicular growth indicates an adequate ovarian response, administer hCG. Follow current clinical practice to reduce the risk of ovarian hyperstimulation syndrome.
Spermatogenesis (males) (off-label use): IM: Following pretreatment with hCG: 75 units 3 times per week with hCG twice weekly until sperm is detected in the ejaculate (4 to 6 months); if response is inadequate after 6 months, may increase Gonapure dosage to 150 units 3 times per week for another 6 months (AACE [Petak 2002]).

Headache; nausea; pain, swelling, heat, or redness at the injection site; stomach cramps, fullness, or pain; vomiting.
Seek medical attention right away if any of these SEVERE side effects occur while taking Gonapure (the active ingredient contained in Gonapure)
Severe allergic reactions (rash; hives; difficulty breathing; tightness in the chest; swelling of the mouth, face, lips, or tongue); breast pain or enlarged breasts; calf, leg, or arm pain, swelling, redness, or tenderness; change in balance; change in color of skin to a bluish color (eg, on the lips, nail beds, fingers, toes); changes in speech or vision; chest, jaw, or arm pain; confusion; coughing up blood; fainting; fast breathing; fast heartbeat; flu-like symptoms; one-sided weakness; pale skin; shortness of breath; sudden, severe nausea or vomiting; unusual sweating; weakness; yellowing of the skin or eyes.

What is the most important information I should know about Gonapure?
Menotrophin is contraindicated in women who have:
1.A high FSH level indicating primary ovarian failure.
2.Uncontrolled thyroid and adrenal dysfunction.
3.An organic intracranial lesion such as a pituitary tumor.
4.The presence of any cause of infertility other than anovulation unless they are candidates for in vitro-fertilization.
5.Abnormal bleeding of undetermined origin.
6.Ovarian cysts or enlargement not due to polycystic ovary syndrome.
7.Prior hypersensitivity to Gonapure.
8.Menotrophin is not indicated in women who are pregnant. There are limited human data on the effects of Gonapure when administered during pregnancy.
