What is Itrin?
Itrin is used to treat high blood pressure (hypertension).
High blood pressure adds to the work load of the heart and arteries. If it continues for a long time, the heart and arteries may not function properly. This can damage the blood vessels of the brain, heart, and kidneys, resulting in a stroke, heart failure, or kidney failure. High blood pressure may also increase the risk of heart attacks. These problems may be less likely to occur if blood pressure is controlled.
Itrin helps to lower blood pressure by relaxing blood vessels so that blood passes through them more easily.
Itrin is also used to treat benign enlargement of the prostate (benign prostatic hyperplasia [BPH]). Benign enlargement of the prostate is a problem that can occur in men as they get older. The prostate gland is located below the bladder. As the prostate gland enlarges, certain muscles in the gland may become tight and get in the way of the tube that drains urine from the bladder. This can cause problems in urinating, such as a need to urinate often, a weak stream when urinating, or a feeling of not being able to empty the bladder completely.
Itrin helps relax the muscles in the prostate and the opening of the bladder. This may help increase the flow of urine and/or decrease the symptoms. However, Itrin will not help shrink the prostate. The prostate may continue to grow. This may cause the symptoms to become worse over time. Therefore, even though Itrin may lessen the problems caused by enlarged prostate now, surgery still may be needed in the future.

Itrin indications
Itrin (Itrin) is indicated for the treatment of symptomatic benign prostatic hyperplasia (BPH). There is a rapid response, with approximately 70% of patients experiencing an increase in urinary flow and improvement in symptoms of BPH when treated with Itrin (Itrin). The long-term effects of Itrin (Itrin) on the incidence of surgery, acute urinary obstruction or other complications of BPH are yet to be determined.
Itrin (Itrin) tablets are also indicated for the treatment of hypertension. Itrin (Itrin) tablets can be used alone or in combination with other antihypertensive agents such as diuretics or beta-adrenergic blocking agents.
How should I use Itrin?
Use Itrin as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- Take Itrin by mouth with or without food.
- Itrin may cause a sudden drop in blood pressure after the first dose. Take your first dose at bedtime. If you get up during the night, sit up slowly, then stand slowly.
- If you miss a dose of Itrin, take it as soon as possible. If it is almost time for your next dose, skip the missed dose and go back to your regular dosing schedule. Do not take 2 doses at once. If more than one dose is missed, contact your doctor or pharmacist.

Benign prostatic hyperplasia: Treatment of symptomatic benign prostatic hyperplasia (BPH)
Hypertension: Management of hypertension. Note: Alpha blockers are not recommended as first line therapy (ACC/AHA [Whelton 2017]).
Off Label Uses
Ureteral calculi expulsion
Use of alpha-1 adrenergic blockers, including Itrin, for treating ureteral calculi is supported by data from a meta-analysis as well as numerous controlled trials. Tamsulosin has the most data, but Itrin has also demonstrated efficacy in facilitating expulsion of ureteral stones (<10 mm) as medical therapy alone or as an adjunct to shock wave lithotripsy and uteroscopy. Some trials indicate a class effect of alpha-1 adrenergic blockers, with Itrin reported to be equally effective as tamsulosin, doxazosin, and alfuzosin.
Itrin description
Itrin (Itrin), an α1-selective adrenoceptor-blocking agent, is a quinazoline derivative. It is piperazine, 1-(4-amino-6,7-dimethoxy-2-quinazolinyl)-4-[(tetra-hydro-2-furanyl)carbonyl]-, monohydrochloride, dihydrate.
Itrin is a white, crystalline substance, freely soluble in water and isotonic saline and has a molecular weight of 459.93.
Inactive Ingredients: Corn starch, lactose, magnesium stearate, povidone and talc. In addition, 2-mg tab contains FD & C Yellow No. 6 and the 5-mg tab contains iron oxide.
Itrin dosage
Adults: Initial Dose: 1 mg at bedtime is the starting dose for all patients and this dose should not be exceeded as an initial dose. Patients should be closely followed-up during initial administration in order to minimize the risk of severe hypotensive response.
Maintenance Dose: Hypertension: Single Dose: 2-10 mg once a day in the morning or evening and should be adjusted to same time as usual. For desired blood pressure response, the dosage may be increased gradually. Doses >20 mg do not appear to provide further blood pressure effect and doses >40 mg have not been studied.
Concomitant administration with thiazide diuretics and other antihypertensive agents: Caution should be observed when Itrin is administered concomitantly with other antihypertensive agents, especially the calcium-channel blocker verapamil, to avoid the possibility of developing significant hypotension. When using Itrin and other antihypertensive agents concomitantly, dosage reduction and retitration of either agent may be necessary.
Benign Prostatic Hyperplasia: Usual Maintenance Dose: 5-10 mg once daily. The dose is increased gradually according to the patient's age and response. To achieve the desired improvement in symptoms as Itrin, subsequent dosage may be increased in a stepwise manner.
Administration: If other drugs are inadequate or ineffective, Itrin can be used alone or in combination with other thiazide diuretics or antihypertensive agents.
Itrin can be administered regardless of meals, because food does not affect bioavailability of Itrin particularly at all.
The dose of Itrin should be adjusted according to blood pressure response and tolerance.

What other drugs will affect Itrin?
Concomitant administration of Itrin (Itrin) with a phosphodiesterase-5 (PDE-5) inhibitor can result in additive blood pressure lowering effects and symptomatic hypotension.
In controlled trials, Itrin (Itrin) tablets have been added to diuretics, and several beta-adrenergic blockers; no unexpected interactions were observed. Itrin (Itrin) tablets have also been used in patients on a variety of concomitant therapies; while these were not formal interaction studies, no interactions were observed. Itrin (Itrin) tablets have been used concomitantly in at least 50 patients on the following drugs or drug classes: 1) analgesic/anti-inflammatory (e.g., acetaminophen, aspirin, codeine, ibuprofen, indomethacin); 2) antibiotics (e.g., erythromycin, trimethoprim and sulfamethoxazole); 3) anticholinergic/sympathomimetics (e.g., phenylephrine hydrochloride, phenylpropanolamine hydrochloride, pseudoephedrine hydrochloride); 4) antigout (e.g., allopurinol); 5) antihistamines (e.g., chlorpheniramine); 6) cardiovascular agents (e.g., atenolol, hydrochlorothiazide, methyclothiazide, propranolol); 7) corticosteroids; 8) gastrointestinal agents (e.g., antacids); 9) hypoglycemics; 10) sedatives and tranquilizers (e.g., diazepam).
Use with Other Drugs
In a study (n=24) where Itrin and verapamil were administered concomitantly, Itrin's mean AUC increased 11% after the first verapamil dose and after 3 weeks of verapamil treatment it increased by 24% with associated increases in Cmax (25%) and Cmin (32%) means. Itrin mean Tmax decreased from 1.3 hours to 0.8 hours after 3 weeks of verapamil treatment. Statistically significant differences were not found in the verapamil level with and without Itrin. In a study (n=6) where Itrin and captopril were administered concomitantly, plasma disposition of captopril was not influenced by concomitant administration of Itrin and Itrin maximum plasma concentrations increased linearly with dose at steady-state after administration of Itrin plus captopril.

In addition to its needed effects, some unwanted effects may be caused by Itrin (the active ingredient contained in Itrin). In the event that any of these side effects do occur, they may require medical attention.

- Dizziness
- Chest pain
- dizziness or lightheadedness when getting up from a lying or sitting position
- fainting (sudden)
- fast or irregular heartbeat
- pounding heartbeat
- shortness of breath
- swelling of feet or lower legs
- Weight gain
Minor Side Effects
Some of the side effects that can occur with Itrin may not need medical attention. As your body adjusts to the medicine during treatment these side effects may go away. Your health care professional may also be able to tell you about ways to reduce or prevent some of these side effects. If any of the following side effects continue, are bothersome or if you have any questions about them, check with your health care professional:

- Headache
- unusual tiredness or weakness
- Back or joint pain
- blurred vision
- drowsiness
- nausea and vomiting
- stuffy nose

What is the most important information I should know about Itrin?
You should not use this medication if you are allergic to Itrin.
Itrin may cause dizziness or fainting, especially when you first start taking it or when you start taking it again. You may wish to take this medication only at bedtime if it causes you to feel light-headed. Be careful if you drive or do anything that requires you to be alert. Avoid standing for long periods of time or becoming overheated during exercise and in hot weather. Avoid getting up too fast from a sitting or lying position, or you may feel dizzy.
If you stop taking Itrin for any reason, call your doctor before you start taking it again. You may need a dose adjustment.
Itrin can affect your pupils during cataract surgery. Tell your eye surgeon ahead of time that you are using this medication. Do not stop using Itrin before surgery unless your surgeon tells you to.
Tell your doctor about all other medications you use, especially other blood pressure medications including diuretics (water pills).
