What is Kwellada?
Treating head lice.
Kwellada cream rinse is a scabicide. It works by killing lice, although this process is not fully understood. It may attack the nervous system of adult lice or interfere with the development of the eggs they produce.
Kwellada indications
Kwellada Cream, 5% is indicated for the treatment of infestation with Sarcoptesscabiei(scabies).
How should I use Kwellada?
Use Kwellada lotion as directed by your doctor. Check the label on the medicine for exact dosing instructions.
- Before using, wash hair with regular shampoo and towel dry.
- Kwellada lotion is for external use only. Do not get Kwellada lotion in the eyes. Keep eyes closed until Kwellada lotion is rinsed out of the hair. If using Kwellada lotion on a child, cover the eyes with a washcloth or towel.
- Apply enough of Kwellada lotion to saturate hair and scalp, especially behind the ears and on the nape of the neck. Leave on hair for 10 minutes, but no longer. Rinse with water.
- If live lice are observed after 7 days or more after the first application, treat hair and scalp a second time.
- Head lice live on the scalp and lay small white eggs (nits) on the hair close to the scalp. For proper head lice management, remove eggs with the comb provided.
- All personal headgear, scarves, coats, and bed linen should be disinfected by machine washing in hot water and drying. Use the hot cycle of a dryer for at least 20 minutes.
- Personal articles of clothing or bedding that cannot be washed or dried by machine may be dry cleaned, sealed in a plastic bag for a period of about 2 weeks, or sprayed with a product specifically designed for this purpose.
- Personal combs and brushes may be disinfected by soaking in hot water above 130 degrees F (54 degrees C) for 5 to 10 minutes.
- Thoroughly vacuum rooms, including furniture, of individuals infected with lice.
- If you miss a dose of Kwellada lotion, use it as soon as you remember. Continue to use it as directed by your doctor.

Uses of Kwellada in details
Kwellada is used to treat scabies (itchy and highly contagious skin infestation by itch mite sarcoptes scabiei), infestation by head lice (head paediculosis) and pubic infestation (paediculosis pubis).
Kwellada description
Kwellada is an epimer of chenodeoxycholic acid. It is a mammalian bile acid found first in the bear and is apparently either a precursor or a product of chenodeoxycholate. Its administration changes the composition of bile and may dissolve gallstones. It is used as a cholagogue and choleretic.
Kwellada dosage
Usual Adult Dose for Scabies
Kwellada topical 5% cream:
Thoroughly massage into the skin from the neck to the soles of the feet. The cream should be removed by washing (shower or bath) after 8 to 14 hours.
Usual Adult Dose for Lice
Kwellada topical 1% kit:
Apply lotion/creme rinse to shampooed and towel-dried hair until hair and scalp are saturated (especially behind the ears and on the nape of the neck). Leave on hair for 10 minutes and rinse with water. Use comb and/or gloves provided to remove remaining nits.
Kwellada topical 1% solution:
Apply to shampooed and towel-dried hair until hair and scalp are saturated (especially behind the ears and on the nape of the neck). Leave on hair for 10 minutes and rinse with water.

Precautions
For external use only. Avoid contact with the eyes and mucous membranes. If contact occurs, wash the area thoroughly with water.
Usually 30 grams of Kwellada cream is sufficient for an average adult.
Scabies rarely infest the scalp of adults, although the hairline, neck, temple, and forehead may be infested in geriatric patients and infants.
One application is generally curative. A second treatment of Kwellada lotion may be given for patients that present with live lice 7 days after the initial treatment.
Patients may experience persistent pruritus after treatment for scabies. Demonstrable living mites after 14 days indicate retreatment is necessary.
Safety and efficacy in children under the age of 2 months have not been established.

Concomitant treatment of individuals in the household is recommended to avoid reinfestation. Thorough cleaning of all clothing and bedding is required once treatment has begun.
For proper head lice management, nits should be removed with a nit comb.
Kwellada interactions
Bile acid sequestering agents such as cholestyramine and colestipol may interfere with the action of Ursodiol by reducing its absorption. Aluminum-based antacids have been shown to adsorb bile acids in vitro and may be expected to interfere with Ursodiol in the same manner as the bile acid sequestering agents. Estrogens, oral contraceptives, and clofibrate (and perhaps other lipid-lowering drugs) increase hepatic cholesterol secretion, and encourage cholesterol gallstone formation and hence may counteract the effectiveness of Ursodiol.

What are the possible side effects of Kwellada?
In clinical trials, generally mild and transient burning and stinging followed application with Kwellada cream, 5% in 10% of patients and was associated with the severity of infestation. Pruritus was reported in 7% of patients at various times post-application. Erythema, numbness, tingling, and rash were reported in 1 to 2% or less of patients. Other adverse events reported since marketing Kwellada cream, 5% include: headache, fever, dizziness, abdominal pain, diarrhea and nausea and/or vomiting. Although extremely uncommon and not expected when used as directed, rare occurrences of seizure have been reported. None have been medically confirmed as associated with Kwellada Cream, 5% treatment.

What is the most important information I should know about Kwellada?
Kwellada Lotion is contraindicated for premature neonates because their skin may be more permeable than full term infants and their liver enzymes may not be sufficiently developed. It is also contraindicated for patients with Norwegian (crusted) scabies due to possible increased absorption. It is also contraindicated for patients with known seizure disorders and for individuals with a known sensitivity to the product or any of its components.
