For treatment of moderate to severe vasomotor symptoms associated with the menopause, female hypogonadism, prostatic carcinoma-palliative therapy of advanced disease, breast cancer, as an oral contraceptive, and as emergency contraceptive. (Dysmenorrhea; Hirsutism; Menorrhagia; Osteoporosis postmenopausal; Premenstrual dysphoric disorder; abnormal uterine bleeding; moderate Acne vulgaris; moderate vasomotor symptoms; severe Acne vulgaris; severe vasomotor symptoms;)

Uses of Primogyn C in details

This medication is used to treat cancer of the breast or uterus. Megestrol is similar to a natural substance made by your body called progesterone.

OTHER USES: This section contains uses of this drug that are not listed in the approved professional labeling for the drug but that may be prescribed by your health care professional. Use this drug for a condition that is listed in this section only if it has been so prescribed by your health care professional.

This medication may also be used to treat cancer of the ovaries or prostate. This medication may also be used to treat weight loss in people with certain conditions (such as cancer, AIDS) by improving their appetite.

How to use Primogyn C

Take this medication by mouth as directed by your doctor. The dosage and how often you take this medication are based on your medical condition and response to treatment. Follow your doctor's instructions carefully.

Use this medication regularly to get the most benefit from it. To help you remember, take it at the same time(s) each day.

Do not increase your dose or use this drug more often or for longer than prescribed. Your condition will not improve any faster, and your risk of side effects will increase.

Since this drug can be absorbed through the skin and lungs and may harm an unborn baby, women who are pregnant or who may become pregnant should not handle this medication or breathe the dust from the tablets.

Primogyn C description

A semisynthetic alkylated estradiol with a 17-alpha-ethinyl substitution. It has high estrogenic potency when administered orally and is often used as the estrogenic component in oral contraceptives [PubChem]. Primogyn C is marketed mostly as a combination oral contraceptive under several brand names such as Alesse, Tri-Cyclen, Triphasil, and Yasmin. The FDA label includes a black box warning that states that combination oral contraceptives should not be used in women over 35 years old who smoke due to the increased risk of serious cardiovascular side effects.

Usual Adult Dose for Postmenopausal Symptoms

10 to 50 mcg orally once a day

Comment:

-Given on a cyclical basis, 3 weeks on and 1 week off.

-Addition of progestogen is not recommended in women without a uterus who did not have endometriosis.

-A progestogen should be added for at least 12 to 14 days every month (28-day cycle) in women with a uterus (or in endometriosis when endometrial foci are present despite hysterectomy).

-Start therapy any time in women with amenorrhea.

-Start therapy on the first day of bleeding in women who are menstruating.

Use: For postmenopausal symptoms due to estrogen deficiency (including prevention of postmenopausal osteoporosis)

Usual Adult Dose for Prostate Cancer

0.15 to 1.5 mg orally once a day

Use: Palliative treatment of prostatic cancer

Usual Adult Dose for Primary Ovarian Failure

10 to 50 mcg orally once a day

Comments:

-Usually given on a cyclical basis.

-Follow initial estrogen by combined estrogen/progestogen.

Use: Hormone replacement therapy (HRT) for failure of ovarian development (e.g., in patients with gonadal dysgenesis where initial estrogen therapy is later followed by combined estrogen/progestogen therapy)

Usual Adult Dose for Menstrual Disorders

20 to 50 mcg orally once a day from day 5 to day 25 of each cycle

Comments:

-A progestogen is given daily throughout the cycle or from days 15 to 25 of the cycle.

-Take a missed dose as soon as it is remembered; if it is nearly time for the next dose then the patient should wait until then. Two doses should not be taken together. Forgetting a dose may increase the likelihood of break-through bleeding and spotting.

Use: For disorders of menstruation (given in conjunction with a progestogen)

DRUG/LABORATORY TEST INTERACTIONS

1. Accelerated prothrombin time, partial thromboplastin time, and platelet aggregation time; increased platelet count; increased factors II, VII antigen, VIII antigen, VIII coagulant activity, IX, X, XII, VII-X complex, II-VII-X complex, and beta-thromboglobulin; decreased levels of anti-factor Xa and antithrombin III, decreased antithrombin III activity; increased levels of fibrinogen and fibrinogen activity; increased plasminogen antigen and activity.

2. Increased thyroid-binding globulin (TBG) levels leading to increased circulating total thyroid hormone levels as measured by protein-bound iodine (PBI), T4 levels (by column or by radioimmunoassay) or T3 levels by radioimmunoassay. T3 resin uptake is decreased, reflecting the elevated TBG. Free T4 and free T3 concentrations are unaltered. Patients on thyroid replacement therapy may require higher doses of thyroid hormone.

3. Other binding proteins may be elevated in serum, (i.e., corticosteroid binding globulin (CBG), sex hormone binding globulin (SHBG)) leading to increased total circulating corticosteroids and sex steroids, respectively. Free hormone concentrations may be decreased. Other plasma proteins may be increased (angiotensinogen/renin substrate, alpha-1-antitrypsin, ceruloplasmin).

4. Increased plasma HDL and HDL2 cholesterol subfraction concentrations, reduced LDL cholesterol concentration, increased triglyceride levels.

5. Impaired glucose tolerance.

6. Reduced response to metyrapone test.

Trouble sleeping, fever, gas, decreased sexual ability/desire, weight gain, changes in appetite, or upset stomach may occur. Women may have changes in their menstrual periods, including unpredictable bleeding. If any of these effects last or get worse, tell your doctor or pharmacist promptly.

Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects. Many people using this medication do not have serious side effects.

Tell your doctor right away if you have any serious side effects, including: fast/pounding heartbeat, fatigue, headache, swelling of the hands/feet.

This medication may rarely make your blood sugar level rise, which can cause or worsen diabetes. Tell your doctor right away if you develop symptoms of high blood sugar, such as increased thirst and urination. If you already have diabetes, be sure to check your blood sugars regularly. Your doctor may need to adjust your anti-diabetic medication, exercise program, or diet.

Get medical help right away if you have any very serious side effects, including: symptoms of a blood clot (such as chest pain, trouble breathing, pain/redness/swelling of arms or legs), confusion, mental/mood changes, weakness/numbness on one side of the body, trouble speaking, vision changes.

A very serious allergic reaction to this drug is rare. However, get medical help right away if you notice any symptoms of a serious allergic reaction, including: rash, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing.

Primogyn C contraindications

ENJUVIA tablets should not be used in individuals with any of the following conditions:

1. Undiagnosed abnormal genital bleeding.

2. Known, suspected, or history of cancer of the breast.

3. Known or suspected estrogen-dependent neoplasia.

4. Active deep vein thrombosis, pulmonary embolism or a history of these conditions.

5. Active or recent (e.g., within the past year) arterial thromboembolic disease (e.g., stroke, myocardial infarction).

6. Liver dysfunction or disease.

7. ENJUVIA tablets should not be used in patients with known hypersensitivity to its ingredients.

8. Known or suspected pregnancy. There is no indication for ENJUVIA in pregnancy. There appears to be little or no increased risk of birth defects in children born to women who have used estrogens and progestins from oral contraceptives inadvertently during early pregnancy.
