What is Clobex Shampoo?

Clobex Shampoo lotion is a prescription corticosteroid medicine used on the skin (topical) for the relief of swelling (inflammation) and itching caused by certain skin conditions in people 18 years of age and older.

Clobex Shampoo lotion should not be used for the treatment of acne vulgaris, rosacea or red, dry itchy skin (dermatitis) around the mouth.

Clobex Shampoo lotion is not recommended in children under 18 years of age.

Important: Clobex Shampoo lotion is for use on the skin only. Do not get the lotion near or in your eyes, mouth or vagina.

Indication

Clobex Shampoo® Spray, 0.05% is a super-high potent topical corticosteroid formulation indicated for the treatment of moderate to severe plaque psoriasis affecting up to 20% body surface area (BSA) in patients 18 years of age or older. The total dosage should not exceed 50 g (59 mL or 2 fl. oz.) per week. Do not use more than 26 sprays per application or 52 sprays per day. Treatment should be limited to 4 consecutive weeks.

Patients should be instructed to use Clobex Shampoo® Spray, 0.05% for the minimum amount of time necessary to achieve the desired results see [Dosage and Administration (2)].

Use in patients under 18 years of age is not recommended because safety has not been established and because numerically high rates of HPA axis suppression were seen with other clobetasol propionate topical formulations.

Limitations of Use

Clobex Shampoo® Spray, 0.05% should not be used on the face, axillae, or groin. Clobex Shampoo® Spray, 0.05% should not be used if there is atrophy at the treatment site. Clobex Shampoo® Spray, 0.05% should not be used in the treatment of rosacea or perioral dermatitis.

Clobex Shampoo is used to treat itching, redness, dryness, inflammation, and discomfort of various scalp and skin conditions, including immune-mediated skin disease with red scaly patches (psoriasis).

Clobex Shampoo description

Clobex Shampoo scalp application also contains the following excipients: Carbomer, isopropyl alcohol, sodium hydroxide and purified water.

Clobex Shampoo cream also contains the following excipients: Glyceryl monostearate, cetostearyl alcohol, chlorocresol, sodium citrate, monohydrated citric acid, purified water, Arlacel 165, beeswax substitute 6621 and propylene glycol.

Clobex Shampoo ointment also contains the following excipients: Propylene glycol, white soft paraffin and sorbitan sesquioleate.

Apply a thin layer of Clobex Shampoo® Cream or Ointment to the affected skin areas twice daily and rub in gently and completely.

Clobex Shampoo® Cream and Ointment are super-high potency topical corti-costeroids; therefore, treatment should be limited to 2 consecutive weeks and amounts greater than 50 g/week should not be used.

As with other highly active corticosteroids, therapy should be discontinued when control has been achieved. If no improvement is seen within 2 weeks, reassessment of diagnosis may be necessary.

Clobex Shampoo® Cream and Ointment should not be used with occlusive dressings.

Geriatric Use: In studies where geriatric patients (65 years of age or older, see PRECAUTIONS) have been treated with Clobex Shampoo® Cream or Ointment, safety did not differ from that in younger patients; therefore, no dosage adjustment is recommended.

Aldesleukin: Corticosteroids may diminish the antineoplastic effect of Aldesleukin. Avoid combination

Clinical Trials Experience

Because clinical trials are conducted under widely varying conditions, adverse reaction rates observed in the clinical trials of a drug cannot be directly compared to rates in the clinical trials of another drug, and may not reflect the rates observed in practice.

Clobex Shampoo Cream was evaluated in two randomized, multicenter, prospective, vehicle-controlled clinical trials in subjects with moderate to severe plaque psoriasis. Subjects applied Clobex Shampoo Cream or vehicle cream twice daily for 14 days. A total of 354 subjects applied Clobex Shampoo Cream and 178 subjects applied vehicle.

The adverse reaction that occurred in at least 1% of subjects treated with Clobex Shampoo Cream and at a higher incidence than in subjects treated with vehicle cream was application site discoloration (2% versus 1%).

Less common local adverse events occurring in < 1% of subjects treated with Clobex Shampoo Cream were application site atrophy, telangiectasia and rash.

Postmarketing Experience

The following adverse reactions have been identified during post-approval use of clobetasol propionate: striae, irritation, dryness, acneiform eruptions, hypopigmentation, perioral dermatitis, allergic contact dermatitis, secondary infection, hypertrichosis, and miliaria. Because these reactions are reported voluntarily from a population of uncertain size, it is not always possible to reliably estimate their frequency or establish a causal relationship to drug exposure.

Hypersensitivity to clobetasol propionate or to any of the excipients of Clobex Shampoo.

Scalp Application: Infections of the scalp.

Cream/Ointment: Primary skin lesions caused by infection with fungi or bacteria; primary cutaneous viral infections (eg, herpes simplex, chickenpox); rosacea; acne vulgaris; pruritus without inflammation; perianal and genital pruritus; perioral dermatitis.

Use in children: Clobex Shampoo is contraindicated in children <1 year.

Children are more likely to develop local and systemic side effects of topical corticosteroids and in general, require shorter courses and less potent agents than adults.

Care should be taken when using Clobex Shampoo to ensure the amount applied is the minimum that provides therapeutic benefit.
